CCFs fall into two distinct types with different causes and typical patients:
Direct CCF — a tear directly connects the carotid artery to the cavernous sinus. The large majority (70–90%) follow head trauma — a blow to the head, road accident, skull fracture, or occasionally surgery or a ruptured aneurysm. Direct CCFs tend to present suddenly and more dramatically, and are slightly more common in men.
Indirect CCF — develops spontaneously, through smaller and more irregular connections rather than a single tear. These are more common in postmenopausal women, particularly those with high blood pressure or underlying connective tissue or vascular disease, and tend to present more gradually.
Diagnosis starts with an eye examination, followed by imaging. MRI or CT typically shows an enlarged superior ophthalmic vein and an enlarged cavernous sinus — early clues pointing toward CCF rather than a primary eye condition. The definitive study is a cerebral angiogram: a catheter-based examination of the brain's blood vessels that shows exactly where the fistula connects the artery to the vein, and maps the surrounding vessels. This is what allows the treatment to be planned precisely before the procedure itself.
Left untreated, a CCF isn't just uncomfortable — the sustained pressure can progress to permanent vision loss, and in some cases to bleeding inside the brain. Because of this, definitive treatment is recommended in most diagnosed cases rather than watchful waiting, with the choice of approach guided by symptom severity, the angiographic findings, and the risk of intracranial haemorrhage.
